 <title>Learning about forms</title>

 <description>&lt;p&gt;Are there any books out there that teach, or atleast, review the process of making forms (application forms, hospital patients form, surveys. Basically anything that needs to be filled out)?&lt;/p&gt;
&lt;p&gt;Can the design of such forms be considered information design?&lt;/p&gt;
